Diseases of the ileum such as Crohn’s Disease or other chronic bowel inflammatory conditions also cause malabsorption of vitamin B12

Pernicious anemia leads to Vitamin B12 deficiency due to the loss of Intrinsic Factor. This is the end stage of an auto-immune gastritis. If untreated, megaloblastic anemia and neurological complications develop. Pernicious anemia is treated with Vitamin B12 injections, or large doses of oral vitamin B12.

Some medications are thought to interfere with the absorption or metabolism of vitamin B12. These include proton pump inhibitor (PPI) medications, metformin, nitrous oxide anesthesia, some epileptic medications, and colchicine.
